Health and Wellbeing Advice

Our Health Navigation Service aims to provide holistic, tailored support and health advocacy, particularly to those struggling with physical and mental health challenges, to break down barriers to clinical support to enable access to appropriate interventions.

The Health Navigator will assess your wellbeing and support needs and will work with yourself and primary and secondary care service providers to provide the emotional, social and practical support that you require. The role will prioritise existing service users that have been identified as needing the service.

What we do

• Signpost you to organisations in your local area who will be able to help you receive appropriate support.
• Make referrals to other organisations who are specialists in the area where you require support.
• Provide health advocacy for anyone wishing to liaise with primary and secondary care service providers e.g. to cancel/ reschedule a hospital/ GP appointment, to obtain test results.
• Raise safeguarding concerns where we are worried about your safety or someone you live with.

What we don’t do

• Give personal recommendations relating to domiciliary care (agencies that help you live independently at home).
• Provide nursing care (e.g. dress wounds, administer vaccinations, do blood pressure checks or prescribe any medication).
• Manage challenging presentations that cannot be managed in open settings e.g. forensic mental health or mental health symptoms that needs inpatient treatment or care.

Criteria for accessing the service

• Clients over the age of 18
• Live in any of the 32 boroughs across London
• Must be registered with a GP (or about to register)
• If you struggle to manage your own physical or mental health needs, please bring a friend or carer with you when you visit the Centre or any of our group activities

• NHS 111- for non-urgent queries relating to your
• MIND Camden (available in other boroughs too)- offering help and support to those who are struggling to manage their mental wellbeing- 0207 911 0822
• Samaritans- offering a listening ear for anyone who is struggling- 116 247 (available 24/7, 365 days a year)
• ICAP (Irish Counselling and Psychotherapy)- the only British-based counselling and psychotherapy service supporting people from the Irish Community- 0207 272 7906
• Change Grow Live- a free, open access service for anyone living in Camden over 18 affected by alcohol or drugs- 020 7485 2722
